Start with a quick, careful assessment focused on ruling in or out a cardiac cause. Check airway, breathing, and circulation, and monitor vital signs. Look for red flags such as central chest pressure or squeezing, pain radiating to the arm or jaw, sweating, nausea, shortness of breath, or faintness. If any of these are present or the person has risk factors for heart disease (age, smoking, high blood pressure, diabetes, prior heart problems), treat as possible cardiac ischemia and arrange urgent EMS evaluation. Do not delay transport or dismiss the symptoms as merely anxiety.

Provide supportive care while awaiting help, and use appropriate measures (like giving aspirin if not contraindicated) only as guided. Do not rely on a blanket assumption of panic, and avoid giving medications like beta-blockers or sedatives without medical direction, or sending the person to sleep, which could delay critical assessment and treatment. The priority is to identify and escalate potential cardiac issues promptly.
